When someone is ready to leave hospital, getting the right support at the right time can make a big difference. Sometimes people experience delays before they can return home, and we want to understand what that feels like for patients and for the family members or friends who support them.

This survey is for:

  • people aged 18 or over who were discharged from hospital to their home in the last 12 months and experienced a delay
  • carers, family members, or friends who supported someone in this situation

Healthwatch Nottingham & Nottinghamshire is carrying out this survey on behalf of Nottingham Community and Voluntary Service to find out how to make the discharge process smoother, reduce stress for patients and carers, and make sure people get the support they need once they are back home. Your answers will help us understand where local voluntary and community organisations might be able to offer extra help, and how the NHS and the voluntary sector can work better together to improve people’s health and wellbeing.

The survey should take about 5 minutes.
